MED 2124W PROVIDING AGE FRIENDLY CARE

The population of the United States is aging. Physicians in virtually all specialties will spend significant amounts of time caring for older adults. This population of patients has unique needs and vulnerabilities. This course will focus on the foundational knowledge which is required to provide high quality care to older patients. It will be organized using the framework of the 4Ms of Age Friendly Care as described by the John A. Hartford Foundation and the Institute for Healthcare Improvement which state that Age Friendly care focuses on Medications, Mentation, Mobility and What Matters to patients. Each week will focus on a different M and will utilize a flipped classroom approach where students will be provided with resources to review to help them to prepare for virtual case discussions via Zoom. In addition, students will be split into small groups at the beginning of the course to create a new model of care to make the healthcare system more age friendly. Students will present their models of care to their classmates in the last week of the course.
